EMG & Rehab Physicians

Our physicians are board certified in Physical Medicine and Rehabilitation and Electrodiagnostic Medicine. They hold teaching positions at the University of Washington and are on active medical staff at Swedish Medical Center. Our practice has served the Puget Sound area since 1965.

Dr. Cynthia Campbell

Dr. Campbell received her BA degree from Swarthmore College and her BS degree from the University of Washington. She attended medical school at the University of Cincinnati and completed her Physical Medicine and Rehabilitation residency at the University of Washington. As a rehabilitation medicine consultant, Dr. Campbell enjoys working with amputees, patients who have had a stroke, and advising and treating patients requiring wound care.

Dr. Thomas Tai Chung

Dr. Chung received his BS degree from and attended medical school at the University of Michigan. He completed his Physical Medicine and Rehabilitation residency at the University of Washington. As a rehabilitation consultant, Dr. Chung enjoys working with patients with musculoskeletal conditions such as repetitive strain disorders, carpal tunnel syndrome, tendonitis, lower back pain, neck pain, sports injuries, motor vehicle injuries, work related injuries, and impairments related to aging.

Dr. Margaret Forgette

Dr. Forgette received her BS degree from the University of Washington. She attended medical school at the University of Washington and completed her Physical Medicine and Rehabilitation residency at the University of Washington. As a rehabilitation medicine consultant, Dr Forgette enjoys working with patients with musculoskeletal conditions such as repetitive strain disorders, carpal tunnel syndrome, tendonitis, lower back pain, neck pain, and work related injuries.

Dr. David Tempest

Dr. Tempest received his BS degree from Oberlin College. He attended medical school at the University of Oregon Health Sciences Center and completed his Physical Medicine and Rehabilitation residency at the University of Washington. As a rehabilitation consultant, Dr Tempest enjoys working with people with neurologic conditions such as stroke, spinal cord injury, brain injury, and multiple sclerosis. He also enjoys working with patients with physical impairments related to aging.
